HCBS Outcomes

As of 2016, OU provides services to 86 individuals: 26 Female & 60 Male. 13 individuals between ages of 0-10; 19 individuals 11-14; 22 individuals 15-18; 26 individuals 19-26; 2 individuals 27-35; 4 individuals are 36-65.

Client diagnoses include: 31 Autism, 9 with Brain Injury, 2 with Mental Illness, 28 with Developmental Disability, 11 wtih Down Syndrome, 7 with CP, 2 with Muscular Dystrophy, and 1 with Spina Bifidia.

Opportunities Unlimited serves individuals on four HCBS Waivers; they are as follows:

  • Brain Injury 11%
  • Intellectual Disabilities 61%
  • Childrens Mental Health 8%
  • Ill and Handicapped 18%
  • Other 2%

Results from the 2016 HCBS satisfaction survey:

  • 84% of persons served were satisfied or very satisfied with the services they receive from OU.
  • Daisy Lane – HCBS
    • 69% reported that they were satisfied or very satisfied with the services provided by Opportunities Unlimited.

Employment Services Outcomes

In our Employment Services program, services were provided to 63 individuals. 44 were male and 19 females. 59 aged 18-40m and 4 aged 41-65. Services accessed by individuals from OU’s Residential program, HCS Pre-Vocational Program, Supported Employment, LIFE Academy, Ticket to Work, and IVRS.

In the Employment Services program, satisfaction surveys were sent to persons served and their families. 95% of the respondents were satisfied with the services that OU provides.
