Opportunities Unlimited’s Community Neurobehavioral Program serves individuals with a brain injury diagnosis as they learn skills needed to integrate into their communities and successfully transition home. The neurobehavioral residential team focuses on teaching strategies and skills for independent living and community integration, while supporting and managing neurobehavioral challenges of the individual.
Brain injury Specialists provide a structured program focusing on neurobehavioral supports and services to individuals. This goal-driven program is individualized to allow all persons served to work on skill building in multiple need areas to ensure a successful transition to their home and communities.
The Cognitive Neurobehavioral home is located in a residential area on the Northside of Sioux City, Iowa. The home is fully accessible for individuals of all mobility levels, allowing persons served to thrive in a home environment supporting their independence. The home includes 4-5 bedrooms which each have their own half bath, a large bathroom with a whirlpool tub and shower, staff office, a recreational room, a large living room area, dining room, an accessible kitchen, laundry facilities, and access to Opportunities Unlimited’s community center for further rehabilitative services.
The Oakshire home was custom built with our persons served in mind, including their comfort, privacy, dignity, and accessibility. The home includes 5 bedrooms, each with a half bath, a large bathroom with a whirlpool tub and shower, staff office, a large living room area, dining room, kitchen, and laundry facilities. The home is located on the north side of Sioux City with close proximity and access to OU’s community center for further rehabilitative services.

All referrals are reviewed on a case-by-case basis by the Admissions and Discharge team to identify appropriateness of the individual for this program.
